High-Intensity Focused Ultrasound- and Radiation Therapy-Induced Immuno-modulation: Comparison and Potential Opportunities

2017

In recent years, high-intensity focused ultrasound (HIFU) has emerged as a new and promising non-invasive and non-ionizing ablative technique for the treatment of localized solid tumors. Extensive pre-clinical and clinical studies have evidenced that, in addition to direct destruction of the primary tumor, HIFU-thermoablation may elicit long-term systemic host anti-tumor immunity. In particular, an important consequence of HIFU treatment includes the release of tumor-associated antigens (TAAs), the secretion of immuno-suppressing factors by cancer cells and the induction of cytotoxic T lymphocyte (CTL) activity. Radiation therapy (RT) is the main treatment modality used for many types of tu…

Hypo-fractionated stereotactic radiation therapy for lung malignancies by means of helical tomotherapy: report of feasibility by a single-center expe…

2018

Background: Several experiences in the literature report SBRT as an effective treatment option for medically inoperable early stage non-small cell lung cancer (NSCLC) and oligometastatic disease. The optimal fractionation schedules and total dose remain controversial. In this study, we evaluated the safety in terms of toxicity and efficacy of using of 8–10 fractions schedules with Helical Tomotherapy (HT) for primary and metastatic lung lesions. Methods: Between March 2014 and May 2016, a total of 39 patients (median age 72 years, range 26–91) were treated with HT-SBRT for malignant lung lesions: 22 patients with early stage NSCLC, 17 with oligometastases. Patients received 8–10 fract…

A Novel Radiotherapeutic Approach to Treat Bulky Metastases Even From Cutaneous Squamous Cell Carcinoma: Its Rationale and a Look at the Reliability …

2022

IntroductionMetastatic cutaneous squamous cell carcinoma (cSCC) is a very rare condition. The lack of definition of an oligometastatic subgroup means that there is no consensus for its treatment, unlike the mucosal head and neck counterpart. Like the latter, the cutaneous form is able to develop bulky tumor masses. When this happens, the classic care approach is just for palliative intent due to a likely unfavorable benefit–risk balance typical of aggressive treatments. Here we proposed a novel radiotherapy (RT) technique to treat bulky metastases from cSCC in the context of an overall limited tumor burden and tried to explain its clinical outcome by the currently available mathematical rad…

SU-E-T-176: Commissiong and Initial Clinical Experience with Dosimetry Check, a Commercial Software for in Vivo Volumetric Dosimetry

2013

Purpose: The aim of this work is to study the differences of reference point doses between the commercial EPID based dosimetry software Dosimetry Check (DC) and TPS to establish an accuracy level of the system, to evaluate its use in clinical routine and to obtain results for the first patients. Methods: We used DC v.3.8 (Math Resolutions), two Varian Clinac iX accelerators equipped with EPID aS1000 and Eclipse v.10.0 with AAA algorithm. Several plans with and without air gap were generated over the phantoms: MP1 Water Tank and Solid Octavius 4D (PTW). 5 head and neck, 3 lung and 4 prostate cases were selected for clinical evaluation. We compared the calculated dose at the isocenter and in …

Evaluation of lens absorbed dose with Cone Beam IGRT procedures

2015

The purpose of this work is to evaluate the absorbed dose to the eye lenses due to the cone beam computed tomography (CBCT) system used to accurately position the patient during head-and-neck image guided procedures. The on-board imaging (OBI) systems (v.1.5) of Clinac iX and TrueBeam (Varian) accelerators were used to evaluate the imparted dose to the eye lenses and some additional points of the head. All CBCT scans were acquired with the Standard-Dose Head protocol from Varian. Doses were measured using thermoluminescence dosimeters (TLDs) placed in an anthropomorphic phantom. TLDs were calibrated at the beam quality used to reduce their energy dependence. Average dose to the lens due to …

SU-E-J-69: Evaluation of the Lens Dose On the Cone Beam IGRT Procedures

2014

Purpose: With the establishment of the IGRT as a standard technique, the extra dose that is given to the patients should be taken into account. Furthermore, it has been a recent decrease of the dose threshold in the lens, reduced to 0.5 Gy (ICRP ref 4825-3093-1464 on 21st April, 2011).The purpose of this work was to evaluate the extra dose that the lens is receive due to the Cone-Beam (CBCT) location systems in Head-and-Neck treatments. Methods: The On-Board Imaging (OBI) v 1.5 of the two Varian accelerators, one Clinac iX and one True Beam, were used to obtain the dose that this OBI version give to the lens in the Head-and-Neck location treatments. All CBCT scans were acquired with the Sta…

Determination of boundaries between ranges of high and low gradient of beam profile

2016

Abstract Aim This work addresses the problem of treatment planning system commissioning by introducing a new method of determination of boundaries between high and low gradient in beam profile. Background The commissioning of a treatment planning system is a very important task in the radiation therapy. One of the main goals of this task is to compare two field profiles: measured and calculated. Applying points of 80% and 120% of nominal field size can lead to the incorrect determination of boundaries, especially for small field sizes. Materials and methods The method that is based on the beam profile gradient allows for proper assignment of boundaries between high and low gradient regions …

Clinical microbeam radiation therapy with a compact source: specifications of the line-focus X-ray tube

2020

Highlights • Line-focus X-ray tubes are suitable for clinical microbeam radiation therapy (MRT). • A modular high-voltage supply safely enables high electron beam powers. • An electron accelerator was designed to generate an eccentric focal spot. • We simulated a peak-to-valley dose ratio above 20 for single-field MRT. • Microbeam arc therapy spares healthy brain tissue compared to single-field MRT.
